White matter is an important component of the central nervous system and is where nerve fibers gather. Damage to the myelin sheath of central nerve cells in the white matter will cause white matter lesions. Neurological symptoms and signs vary, depending on the site and extent of the lesions. Clinically, visual, motor, sensory, cerebellar, autonomic and cognitive dysfunctions may be seen.

Clinical manifestations

The clinical manifestations of white matter lesions are complex and varied, depending on the location and severity of the lesions. The clinical manifestations of demyelinating diseases of different natures vary greatly. For example, multiple sclerosis is mostly a chronic disease, with relapses and remissions in more than half of the cases. The incidence ratio of female to male is 2:1, and the age of onset is usually 20 to 40 years old. About half of the patients present with limb weakness, numbness, or both as the first symptom. Its typical signs include: limb paralysis, visual impairment, nystagmus and eye muscle paralysis, cranial nerve damage, sensory impairment, ataxia, paroxysmal neurological symptoms, cognitive dysfunction, autonomic nerve dysfunction, mental disorder, etc. An inherited myelinating disorder that begins in infancy or childhood and often has features of involvement of other systems.

Treatment for immune-related demyelination can include:

①Corticotropin and corticosteroids, such as methylprednisolone.

②β-interferon therapy.

③Glatiramer acetate.

④Azathioprine.

⑤ Large doses of immunoglobulin intravenous infusion.

Demyelination related to infection, toxicity and metabolism can be treated according to the cause, but there is currently no effective treatment for hereditary demyelination diseases.

Prognosis:

The prognosis of white matter lesions depends on the cause and classification of white matter damage. For example, multiple sclerosis can be divided into bisexual, relapsing-remitting, remittent-progressive, chronic progressive, etc.; about 1/3 of neuromyelitis optica can be completely relieved, and about half of the patients will eventually progress to multiple sclerosis; the mortality rate of patients with acute disseminated encephalomyelitis and central pontine myelinolysis syndrome is relatively high. Patients with hereditary demyelination also have a poor prognosis and a high mortality rate.
