It is necessary to understand the neutrophil alkaline phosphatase score, because a high or low level may be a sign of some diseases. Usually the positive rate is 10-40%, and the score is 40-80. Understanding the NAP positive rate and score can help effectively diagnose and identify some diseases.

1. The normal value range of neutrophil alkaline phosphatase integral is 50~120 points. Its clinical laboratory significance: 1. It helps in the diagnosis and differential diagnosis of chronic myeloid leukemia and infection or leukemoid reaction. 2. It helps in the differential diagnosis of myeloproliferative diseases.

2. Alkaline phosphatase mainly exists in mature neutrophils, while other cells show negative reaction. A positive reaction is the appearance of gray to brown-black granules in the cytoplasm, and the reaction intensity is divided into 5 levels, namely -, +, 2+, 3+, and 4+. The reaction results were expressed as the percentage and integral value of positive reaction cells. After the blood smear is stained, 100 mature neutrophils are observed continuously under an oil immersion mirror, and the percentage of positive reaction cells is recorded as the positive rate; all positive reaction cells are graded from + to 4+ according to their reaction intensity, and the percentage of each level is multiplied by the level, and then added up to get the integral value.

3. Reference value: positive rate 10-40%, integral value 40-80. Neutrophil alkaline phosphatase (NAP) is a marker enzyme of neutrophils, mainly present in mature neutrophils. It is a nonspecific hydrolase released from special granules in the cytoplasm and can catalyze the hydrolysis of monophosphates of various alcohols and phenols under alkaline conditions (pH 9.3-9.6). NAP activity can reflect the maturity and function of mature granulocytes. As the cells mature, the activity of the enzyme gradually increases.

4. When neutrophils are activated, the NAP positivity rate and score increase. Under pathological conditions, changes in NAP activity often help in the diagnosis and differential diagnosis of certain diseases.
