1. Symptoms of otitis media

1. Acute suppurative otitis media

Suppurative otitis media is commonly known as ear damage. It is an inflammation of the middle ear caused by purulent bacterial infection. Its main symptoms are ear pain and pus discharge. Children have more obvious systemic symptoms than adults, and may have fever, vomiting, etc. The consequence is complications. Severe complications include intracranial complications such as meningitis, brain abscess, etc., and other complications include extracranial complications such as labyrinthitis, facial nerve paralysis, etc.

The ears may lose hearing.

2. Chronic suppurative otitis media

Chronic suppurative otitis media refers to chronic suppurative inflammation of the middle ear mucosa, periosteum or deep into the bone. This disease is relatively common in clinical practice, and its main clinical manifestations are intermittent or continuous pus discharge in the ear, tympanic membrane perforation, and hearing loss. In severe cases, it can cause intracranial and extracranial complications.

1. Systemic symptoms: vary in severity. There may be chills, fever, fatigue, and loss of appetite. Children have more severe systemic symptoms, often accompanied by gastrointestinal symptoms such as vomiting and diarrhea. Once the eardrum is perforated, the body temperature will gradually drop and systemic symptoms will be significantly alleviated.

2. Earache: Pain deep in the ear that gradually worsens. If the pain is throbbing or stabbing, it may radiate to the same side of the head or teeth. The ear pain worsens when swallowing and coughing. People with severe ear pain cannot sleep at night and feel irritable and restless. After the eardrum perforated and pus flowed out, the ear pain suddenly subsided.

3. Ear discharge: It is the main symptom of this disease. It may be mucous, mucopurulent or pure purulent. Non-dangerous pus discharge is thinner and has no odor. Although the dangerous type of pus discharge is not much, it is thicker, mostly pure purulent, and accompanied by a foul odor.

4. Hearing loss and tinnitus: You will start to feel stuffy in the ears, followed by a gradual decrease in hearing, accompanied by tinnitus. In patients with severe ear pain, deafness may be ignored. Some patients may also have vertigo, and the deafness may be alleviated after perforation.

5. Deafness: The severity of deafness varies, but it is often a single-ear disease and can be easily overlooked. This type of deafness is often proportional to the progression of the disease, that is, the more severe the lesion, the more severe the deafness. It is usually conductive hearing loss.

2. TCM treatment of otitis media

1. Wind evil stagnates the orifices

[Symptoms] After catching a cold, you may feel fullness or slight pain in your ears, tinnitus and hearing loss, increased spontaneous sound, a retracted eardrum, red and swollen eardrum, or a fluid level. Accompanied by fever, aversion to wind, nasal congestion, runny nose, etc.

[Treatment method] The treatment should be to dispel wind and evil, promote qi and relieve abdominal distension.

[Prescription] Yinqiao Powder combined with Tongqi Powder: honeysuckle, forsythia, platycodon, mint, bamboo leaf, schizonepeta, light black beans, burdock fruit, reed root, chuanxiong, bupleurum, cyperus rotundus. For severe nasal congestion, add magnolia and Xanthium sibiricum. For severe ear swelling and congestion, add patchouli and calamus. If there is fluid accumulation in the tympanic cavity, add Akebia and Alisma. If it is due to wind-cold blocking the lung meridian, the whole body will have severe chills, mild fever, nasal congestion, clear runny nose, stuffy and swollen ears, and hearing loss; the tongue is pale red, the tongue coating is thin white, and the pulse is floating and rapid. The treatment is suitable for clearing the lungs, dispersing cold and opening the orifices. The prescription is San'ao Decoction combined with Xanthium sibiricum Powder with modifications: ephedra, apricot kernel, Xanthium sibiricum, magnolia, angelica dahurica, mint, schizonepeta, saposhnikovia divaricata, and licorice; decocted in water for oral administration.

2. Phlegm and dampness in the ears

[Symptoms] There is a strong feeling of fullness and blockage in the ears, hearing loss, increased spontaneous sound, and a sound of water in the ears when shaking the head. Examination revealed an arc-shaped horizontal line on the eardrum or a bulge on the eardrum. The whole body is often accompanied by heaviness and dizziness, fatigue, bland mouth and abdominal distension; pale tongue with greasy coating, and moist or slippery pulse.

[Treatment method] It is advisable to strengthen the spleen and raise clearness, eliminate dampness and open the orifices.

[Prescription] Buzhong Yiqi Decoction combined with Wuling Powder with modifications: Astragalus, Codonopsis, Atractylodes, Bupleurum, Angelica, Tangerine Peel, Magnolia, Poria, Alisma, Acorus, Plantago, Pueraria, Akebia; decocted in water for oral administration.

3. Qi and blood stasis

[Symptoms] There is a feeling of blockage in the ear, hearing loss, and tinnitus that gradually occurs and does not heal over time. The tympanic membrane is obviously retracted, or thickened, with calcification, adhesion and atrophy; the tongue is dark red and the pulse is sluggish.

[Treatment method] The treatment should be to promote blood circulation and unblock meridians, and open up hearing.

[Prescription] Buyang Huanwu Decoction with modifications: Astragalus, Angelica, Chuanxiong, Peach Kernel, Safflower, Scorpion, Earthworm, Polygonatum, Pueraria, Luffa, Rhizoma Smilacis Glabrae. If accompanied by dizziness, soreness of waist and knees, it is due to weak kidney essence. Liuwei Dihuang Pills or Erlong Zuoci Pills can be added for oral administration.
