Symptoms include those of the primary disease and those caused by pulmonary hypertension. The symptoms of pulmonary hypertension itself are non-specific. Mild pulmonary hypertension may be asymptomatic. As the disease progresses, the following manifestations may occur: (1) Exertional dyspnea: Due to decreased pulmonary vascular compliance, cardiac output cannot increase with exercise. Dyspnea after physical activity is often the earliest symptom of pulmonary hypertension. (2) Fatigue: It is the result of decreased cardiac output and tissue hypoxia. (3) Syncope: It is caused by a sudden decrease in blood supply to the brain, which often occurs after exercise or when standing up suddenly. It can also be caused by a large embolus blocking the pulmonary artery, sudden spasm of the pulmonary arterioles, or arrhythmia. (4) Angina pectoris or chest pain: Due to right ventricular hypertrophy, coronary artery perfusion is reduced, resulting in relative myocardial insufficiency of blood supply. Chest pain may also be caused by hemangioma-like dilatation of the main trunk or main branches of the pulmonary artery. (5) Hemoptysis: Pulmonary hypertension can cause rupture of microaneurysms at the beginning of pulmonary capillaries and lead to hemoptysis. (6) Hoarseness: caused by pulmonary artery dilation compressing the recurrent laryngeal nerve. |
