What should I do if gastric bleeding recurs?

1. Conventional treatment of gastric bleeding:

② Those who vomit blood should fast, while those who have simple black stools can eat liquid food.

③ Those with serious conditions should receive oxygen.

④ Place a gastric tube to aspirate the blood in the stomach, understand the bleeding situation, and infuse drugs.

⑤ Strengthen care to prevent vomitus from being inhaled into the respiratory tract and causing pneumonia or suffocation.

2. General treatment methods:

The diagnosis of acute gastric bleeding is mainly based on acute massive hematemesis and black stools. Generally, the bleeding does not exceed 500 ml, and most of the cases are vomiting blood, but some cases are bloody stool. The patient feels nauseous before vomiting blood, feels the urge to defecate before having bloody stools, and has black eyes, panic attacks, and even fainting after defecation. The patient looks pale, is thirsty, has a rapid and weak pulse, and has low blood pressure. Patients with ulcer disease often experience regular upper abdominal pain, which can be relieved by alkaline drugs.

X-ray barium meal examination is of great significance in the diagnosis of ulcer disease. In addition to the possibility of feeling a mass, gastric cancer patients also have obvious symptoms such as loss of appetite, upper abdominal discomfort, indigestion, and weight loss, and the disease progresses rapidly. X-ray barium meal examination and gastroscopy can help to confirm the diagnosis. 3. Hemostasis treatment methods

(1) Application of hemostatic agents:

① Choose Anluoxue, Hemostatic Min or Hemostatic Aromatic Acid as appropriate and add it to the infusion solution for drip.

② Infuse norepinephrine or thrombin at a concentration of 80 mg/L through the gastric tube.

③ Local hemostasis under gastroscopy can be achieved by spraying hemostatic agents, such as 80 mg/L norepinephrine or thrombin; or injecting hemostatic agents, such as 1-2 mg epinephrine added to 10 ml of 10% saline and injected at multiple points; or high-frequency electrocoagulation hemostasis; or microwave hemostasis; or laser hemostasis.

(2) First aid measures for gastric bleeding:

① Estimation of blood transfusion volume: Mild bleeding is mainly treated with infusion and blood transfusion may not be required temporarily; moderate bleeding requires 400-600ml of blood supplementation; severe bleeding requires 900-1200ml of blood transfusion, or even more.

② In principle, the amount of fluid to be infused should be estimated based on how much goes out. The infusion volume and speed can be adjusted according to the central venous pressure measurement. The fluids used are crystalloids and dextran.

③Correct acid-base and electrolyte balance.
