The enzyme that catalyzes the cleavage of the aromatic ring is homogentisate oxidase, which is normally present in the soluble parts of liver and kidney tissues. This enzyme is highly specific for the breakdown of homogentisate and is not found in tissues other than the liver and kidneys. In patients with this disease, the activity of this enzyme is completely lacking in the liver and kidneys, resulting in the inability of homogentisate to be broken down into acetoacetate and fumarate. This requires the use of other metabolic mechanisms to process homogentisate. The kidneys have a high clearance rate for homogentisic acid, and the renal tubules actively secrete homogentisic acid. Once the kidneys excrete homogentisic acid from the body, it will gradually oxidize to form polymers that can cause urine discoloration.

The mechanism by which homogentisate deposits in tissues and causes ochronosis is still unclear. Homogentisate has a tendency to deposit in the skin and cartilage, where it is bound by physical attraction. In addition, the decomposition products of homogentisic acid can irreversibly bind to connective tissue to form polymers that can cause pigmentation. The tissue becomes brittle after staining and may even break, leading to degenerative changes in the intervertebral discs and joints. In addition, homogentisate can also directly act on collagen synthesis by inhibiting lysyl hydroxylase.

Pathology: Blackening of connective tissue and cartilage is the basic pathological change of this disease. Homogentisate can be deposited in organs such as the skin, cornea, cartilage, tendons, ligaments, endothelium of large blood vessels, endocardium, thyroid gland, lungs and kidneys, causing the tissues of these organs to turn black. If it involves the cartilage, it can lead to exfoliation of the subchondral bone.
