Treatment of primary liver cancer

Early treatment is the most important factor in improving the prognosis of primary liver cancer. Early primary liver cancer should be surgically removed as much as possible, and multimodal comprehensive treatment can also be used for primary liver cancer that cannot be removed.

The best treatment for primary liver cancer is radical resection. For liver cancer that cannot be removed, surgery or non-surgical comprehensive therapy can be used to shrink the tumor and then perform a second-stage or two-step resection, or to slow down tumor development and prolong survival. Some types of small primary liver cancer can be cured through various non-surgical local treatments. When patients with advanced primary liver cancer cannot tolerate various treatments, they should focus on protecting the liver, improving their overall condition, and symptomatic treatment to relieve pain and improve their quality of life. Comprehensive measures such as surgery, chemotherapy, radiotherapy, traditional Chinese medicine, immunotherapy, and other supportive therapies, symptomatic treatment, etc. should be selected from a holistic perspective and reasonably selected according to the condition of patients with primary liver cancer to achieve the goal of improving efficacy.

There are many treatments for primary liver cancer, mainly:

1. Cryotherapy for liver cancer: Currently it is suitable for tumor nodules with a diameter > 3cm, without severe cardiopulmonary and liver dysfunction, without bleeding tendency, without large amounts of pleural effusion or ascites or extensive metastasis, and without involving the porta hepatis or inferior vena cava.

2. Ultrasound-guided percutaneous puncture local treatment of the tumor: Intratumoral injection is highly safe, has mild side effects, and can be performed repeatedly. Among them, anhydrous ethanol injection (pEI) has been widely used in clinical practice and is suitable for patients with tumors less than 5 cm in diameter, generally no more than 3 lesions, tumors located near large blood vessels in the portal area, patients with poor general condition or recurrence after resection who cannot tolerate surgery.

3. Percutaneous intratumoral radiofrequency ablation (RFA): It is mainly suitable for liver cancer with a diameter of less than 3 cm, number of lesions <4, no portal vein tumor thrombus or extrahepatic metastasis. The complete tumor necrosis rate can reach more than 90%, which is close to the effect of surgical resection.

4. Others: Liver cancer is not very sensitive to radiotherapy and chemotherapy. It can be treated with comprehensive treatments such as biological therapy, immunotherapy and traditional Chinese medicine.

Dietary considerations for primary liver cancer

1. Daily diet should be regular, quantitative, and small and frequent meals to reduce the burden on the gastrointestinal tract.

2. Eat more foods containing vitamins A, C, and E, and eat more green vegetables and fruits.

3. Eat foods that have anti-cancer effects regularly, such as kale, cabbage, carrots, rapeseed, vegetable oil, fish, etc.

4. Stick to low-fat, high-protein and easily digestible foods, such as lean meat, eggs, yogurt, fresh fruit juice and fresh vegetable juice.

5. Eat fresh food and avoid eating moldy or spoiled food.

6. To maintain smooth bowel movements, patients with constipation should eat foods rich in fiber and drink some honey every day.

7. The main foods should include: milk, eggs, soy milk, lotus root powder, fruit juice, vegetable juice, lean meat paste, liver paste, etc.
