Mid-term symptoms of endometrial cancer

Endometrial cancer is a group of epithelial malignant tumors that occur in the endometrium and are more common in perimenopausal and postmenopausal women. Endometrial cancer is one of the most common tumors of the female reproductive system, with nearly 200,000 new cases each year, and is the third most common gynecological malignant tumor leading to death (after ovarian cancer and cervical cancer). Its incidence is closely related to lifestyle, and the incidence rate varies in different regions. In North America and Europe, its incidence rate is second only to breast cancer, lung cancer, and colorectal cancer, ranking first among female reproductive system cancers. In my country, with the development of society and the improvement of economic conditions, the incidence of endometrial cancer has also increased year by year. It is currently second only to cervical cancer in female reproductive system malignant tumors.

1. Vaginal bleeding

It is the most important clinical symptom of endometrial cancer. It manifests as:

(1) Postmenopausal vaginal bleeding: 70% of endometrial cancer patients are postmenopausal women, and more than 90% have vaginal bleeding. It manifests as continuous or intermittent vaginal bleeding, with varying amounts. Generally, the amount of bleeding is not much, and heavy bleeding is rare. There are also cases where heavy bleeding suddenly occurs several years after menopause.

(2) Perimenopausal vaginal bleeding: manifested as menstrual cycle disorder, prolonged menstruation or increased menstrual flow, or irregular vaginal bleeding. About 20% of endometrial cancer patients are perimenopausal women.

2. Vaginal discharge

About 1/3 of patients have increased vaginal discharge, which manifests as serous or bloody secretions. When combined with intrauterine infection or pyometra, the discharge may be purulent or bloody, and sometimes have a foul odor.

3. Pain

Usually it does not cause pain. A small number of patients have lower abdominal pain, which is persistent pain or distending pain, which may be related to the large lesion protruding into the uterine cavity and stimulating uterine cavity contracture. The pain is more obvious when there is fluid accumulation or pus accumulation in the uterine cavity or when advanced cancer infiltrates surrounding tissues or compresses nerves. It manifests as persistent pain in the lower abdomen, lumbar sacral region and legs.

4. Systemic symptoms

Patients with endometrial cancer generally have obesity, diabetes, and hypertension. In the early stages of gynecological examinations, pelvic examinations are usually normal, but in the late stages, there may be signs of uterine enlargement, adnexal masses, anemia, weight loss, cachexia, and distant metastasis.
