What are the general classifications of colorectal cancer

1. Early colorectal cancer: Cancer that is limited to the colorectal mucosa and submucosa is called early colorectal cancer. Generally, there is no lymph node metastasis. However, among those infiltrating into the submucosa, 5% to 10% of cases have local lymph node metastasis. Early colorectal cancer is divided into 3 types based on naked eye observation.

(1) Polypoid type: The appearance shows localized protrusion of the mucosa, which may be pedunculated, subpedunculated, or broad-based. This type is often intramucosal cancer.

(2) Flat raised type: The mucosa is slightly thick, almost normal, with no protrusions on the surface, or slightly raised, like a coin.

(3) Flat bulge with ulceration; like a small disc, with raised edges and a sunken center. Only seen in submucosal cancer.

2. Advanced colorectal cancer refers to cancerous tissue that invades below the mucosal layer and reaches the serous layer. It can be divided into three categories by naked eye observation.

(1) Mass type: mainly grows into the cavity, presents a spherical or hemispherical shape, and has many small ulcers that bleed easily. This type has low invasiveness, late lymph node metastasis, and a better prognosis.

(2) Ulcerative type: It appears as a flat mass at first, which then necrotizes in the center and forms a large ulcer with the edges turned outward in a butterfly shape. The surface is prone to bleeding and infection.

(3) Infiltrative type: Cancerous tissue mainly infiltrates and grows along the intestinal wall, with obvious fibrous tissue reaction, causing intestinal stenosis and intestinal obstruction, early lymph node metastasis, and poor prognosis.
