(3) X-ray examination can determine the location, degree, direction of dislocation, and whether there is fracture or displacement.

1. Concomitant Diseases

This disease is most often accompanied by fractures, nerve damage, etc. About 30 to 40% of cases of shoulder dislocation are accompanied by greater tuberosity fractures. Fractures of the surgical neck of the humerus or compression fractures of the humeral head may also occur. Sometimes, the joint capsule or the glenoid rim is torn off from the anterior attachment. Poor healing may cause habitual dislocation. The long head of the biceps tendon may slip backwards, causing joint reduction problems. The axillary nerve or medial bundle of the brachial plexus may be compressed or pulled by the humeral head, causing nerve dysfunction and may also damage the axillary artery. Therefore, during the inspection, attention should be paid to whether there are other associated injuries.

This phenomenon often occurs in babies under 5 years old. The radial head of the baby is not developed, and the annular ligament of the elbow cannot tightly wrap around the radial head. When parents pull the baby's arm, the radial head is very likely to dislocate. If dislocation occurs repeatedly, it is very likely to become a habitual dislocation.

The baby did not fall and suddenly developed elbow pain and was afraid to move. Most of the time, this occurred after the parents pulled the baby's wrist. If the baby cannot speak, he will suddenly start crying non-stop. If the baby can speak, in addition to crying, he will also complain about the pain in his arm. The mother can see that the baby's arms are often in a semi-flexed position (drooping), the forearms dare not move, the baby cannot pick up any objects with his hands, and cannot raise his arms. There was no obvious swelling or deformity in the arm.

What the doctor does

1. An experienced doctor can quickly reposition the elbow. The specific operation method is: hold the baby upright, and the doctor bends the baby's elbow 90 degrees. The doctor holds the lower end of the baby's upper arm with one hand and places the thumb on the radial head; holds the baby's wrist of the affected limb with the other hand and performs forearm rotation with appropriate traction force. At this time, a crisp, slight snapping sound can be heard or the fingers can be felt bouncing, indicating that the radial head has been repositioned. After reduction, the baby's elbow pain disappears immediately, and the baby can use the affected hand to lift objects.

2. For babies who have been dislocated for more than 24 hours or have a history of repeated dislocation, due to local swelling, the popping sound or bouncing sensation during reduction is often not obvious, and the pain may not disappear immediately after reduction. Most other symptoms can be relieved. At this time, use a neck wristband (or fold your mother's scarf into a triangle) to fix the elbow at a right angle for about a week.

How to care for a child with a dislocated joint

1. Fixing the dislocated part is the best way to relieve pain.

If the baby's arm is immobile on one side but there is no pain, you can use a triangular bandage to slightly fix the dislocated part and then send him to the hospital for examination. If you don't have a scarf, you can put your hands between the buttons of your shirt to secure it.

2. When the baby is dislocated, do not rub the injured part and do not move the baby's injured arm at will.

To avoid causing secondary injury to the baby's injured part during movement, first fix the injured part and then apply ice, and go to the hospital for treatment as soon as possible.

3. Baby dislocation is usually treated by manual reduction, without any anesthesia.

During treatment, parents just need to cooperate with the doctor according to his/her instructions. After reduction, the baby's pain disappears and the injured part can be moved immediately.

4. For babies who have been dislocated for more than 24 hours or have a history of repeated dislocation, the doctor will recommend using a neck wrist brace to fix the elbow at a right angle for about 1 week. Because of local swelling, the pain may not disappear immediately after reduction, but most other symptoms can be relieved. Do not wrap the baby's injured part tightly. So as not to hinder blood return and cause palm edema.
