1. Half of the patients have acute urinary retention, frequent urination, dysuria, rectal discomfort, urethral pus, and some are accompanied by epididymitis. Rectal examination shows that the prostate on the affected side is enlarged, soft to the touch, and has a sense of fluctuation. Occasionally, a prostatic abscess may spontaneously rupture into the urethra or rectum and be mistaken for a perirectal abscess.

3. Prostate stones: refers to stones that occur in the prostate acini and glandular ducts. It is related to factors such as chronic prostate inflammation, prostate fluid retention, glandular duct stenosis, and metabolic disorders. Inorganic salts such as calcium oxalate, calcium phosphate, and magnesium phosphate are deposited on the amyloid bodies, epithelial cells, and inflammatory exudates in the prostate alveoli to form stones. Patients may show various symptoms of chronic prostatitis, but rectal digital examination can feel the friction of stones in the prostate. Pelvic X-ray has a positive stone shadow on one side of the pubic symphysis area. Ultrasound examination can show a strong light band and sound shadow at the site of the prostate stone.

4. Prostatic tuberculosis: Symptoms are similar to those of chronic prostatitis, but there is often a history of urinary tract tuberculosis or tuberculosis lesions in other parts of the body. Rectal examination shows irregular nodules in the prostate, enlarged and hardened epididymis, beaded nodules in the vas deferens, and direct smear or PCR detection of Mycobacterium tuberculosis in prostatic fluid.

5. Prostate cancer: In the late stage, symptoms such as frequent urination, painful urination, and difficulty urinating may appear, but patients often have obvious systemic symptoms such as weight loss, fatigue, anemia, and loss of appetite. Rectal examination of the prostate reveals a hard lump like a rock with an uneven surface, and serum prostate-specific antigen and prostatic acid phosphatase are increased. Prostate puncture biopsy can detect cancer cells, and ultrasound examination can show that the gland is enlarged, the boundary echo is irregular or defective, the internal light spots are uneven, and there are brighter spots or light clusters in the cancerous area. CT examination shows that the prostate is asymmetrical in shape. If the tumor infiltrates outside the capsule, the tissue gap between the seminal vesicle and the posterior wall of the bladder may disappear. CT can determine the extent of prostate cancer invasion.

6. Osteitis pubis: Clinically, it often presents with symptoms of chronic prostatitis, but rectal examination and prostatic fluid examination are normal. The main features are obvious tenderness at the pubic symphysis. Pelvic X-rays show that the pubic symphysis gap is widened by >10 mm, the level difference between the bilateral superior pubic branches is >2 mm, the edge of the pubic symphysis is irregular, and erosion and reactive bone sclerosis occur.
