Left maxillary frontal process fracture

It is inevitable that we will bump into things in life, or we may bump our noses during fights. When the nose is injured, in addition to nasal bone fractures, maxillary frontal process fractures are also common. You can go to the relevant department for a nasal bone CT scan to find out. This type of fracture is usually not serious and generally does not require treatment and will heal on its own. If the dislocation is serious, go to the hospital for examination immediately or have surgery to reduce it.

Left maxillary frontal process fractures are classified as:

1. Lefort Classification: by Rene Le

Fort (1901) proposed three types:

Le Fort I type: a horizontal fracture of the alveolar process base, with the fracture line passing through the lower edge of the piriform foramen, the alveolar process base, around the zygomatic alveolar ridge and maxillary tuberosity to the pterygoid process;

Le Fort II type: maxillary central cone fracture, the fracture line extends from the root of the nose to both sides, through the lacrimal bone, the infraorbital margin, the zygomaticomaxillary suture, around the lateral wall of the maxilla and backward to the pterygoid process;

Le Fort III type: a high horizontal fracture, the fracture line passes through the nasofrontal suture, across the orbit, and then through the zygomaticofrontal suture backward and downward to the pterygoid process, forming craniofacial separation.

2. The improved classification is divided into the following four types:

(1) Low (horizontal) fracture: Le Fort I type horizontal fracture of the maxilla. The main clinical manifestation is relationship disorder. The principle of treatment is to restore the relationship.

(2) High (horizontal) fracture: The maxillary fracture line is at the level of Le Fort type II and/or type III. Clinical manifestations include relationship disorders and facial deformities. The principle of treatment is to restore the facial relationship and correct facial deformities at the same time.

(3) Sagittal fracture: The maxillary bone is broken vertically, with the fracture line located in the middle or paramedian direction. The clinical manifestations are widening of the dental arches, possible opening of the teeth, and fractures that may injure the skull base. The treatment principle is to resolve the palate relationship and close the traumatic cleft palate.

(4) Alveolar process fracture: The fracture line is limited to the apical level and only affects the tooth bone segment. The principle of treatment is to reposition and fix the tooth-bone segments.

What should I do if I have a left maxillary frontal process fracture?

1. Reset

An important sign of mandibular fracture reduction is to restore the normal occlusion relationship of the upper and lower teeth. Common reset methods are:

(1) Manual reduction: In the early stages of mandibular fracture, the fractured segments are relatively mobile and can be restored to their normal position by manipulation.

(2) After traction reduction of mandibular fracture, after a long period of time (more than three weeks for maxillary fracture and more than four weeks for mandibular fracture), some fibrous tissue at the fracture site has healed. If manual reduction is unsuccessful, traction reduction can be used.

(3) Open reduction The indications for open reduction are relatively broad. If the fractured segment has been displaced for a long time and fibrous healing or bony dislocation healing has occurred, and manipulation and traction cannot reduce it, surgical incision and reduction should be performed.

2. Fixation

Commonly used fixation methods include single-jaw dental arch splint fixation, intermaxillary fixation, intermaxillary ligation fixation, small steel plate or micro-steel plate fixation, craniomandibular fixation, and other methods include perimaxillary fixation, compression plate fixation, etc.
