Wheezing is a sign of an asthma attack, during which the bronchial walls become inflamed and swollen, producing heavy, sticky secretions. Phlegm accumulates in the bronchi, causing the smooth muscles of the bronchial walls to contract, resulting in difficulty and discomfort in breathing. The patient will then emit a high-frequency, sharp sound, known as wheezing. During an asthma attack, prolonged exhalation time and wheezing may be heard by auscultating the lungs. Wheezing is a typical auscultatory feature of bronchial asthma.

Why does wheezing occur during an asthma attack?

When bronchial asthma attacks, wheezing is inevitable. At this time, in addition to the widespread spasm of the bronchioles, if there is also swelling of the bronchiolar mucosa and retention of secretions in the lumen, the intensity of the wheezing will be greater. Clinically, widespread and obvious wheezing is one of the important signs for diagnosing bronchial asthma and wheezing chronic bronchitis.

The occurrence of wheezing is mainly due to extensive bronchiolar spasm in the lungs. During exhalation, the airflow passes through the narrow bronchiolar lumen and produces a pathological respiratory sound, which is more obvious during exhalation.

Wheezing is characterized by a high pitch, a musical sound like a vibrating wire, a long duration, and being obvious during exhalation but basically disappearing during inhalation. This pathological breath sound may also occur when ventilation obstruction is caused by other diseases.

Wheezing may also occur during an attack of cardiac asthma, but this wheezing occurs during both inhalation and exhalation, and the extension of exhalation time is far less obvious than in bronchial asthma.
