How to treat kidney stones in TCM?

1. Etiology and pathogenesis:

(1) The main etiology and pathogenesis of stranguria caused by damp-heat accumulation is the accumulation of damp-heat, and stone stranguria is no exception. Usually, people eat a lot of spicy, hot, fatty and sweet foods, or drink too much alcohol, which will lead to damp-heat, which will flow into the lower part of the body and boil the urine. Over time, the impurities in the urine will solidify into sand and stones, which is stone stranguria. The Golden Chamber Essentials describes it as "like simmering sea water to produce salt and alkali."

(2) The formation of stranguria caused by qi stagnation and blood stasis is undoubtedly an important factor in addition to being related to dampness and heat. If the body's qi and blood flow smoothly, the qi, blood and water flow continuously, moving without stopping, and tangible objects cannot gather and cause trouble. Once certain factors cause qi stagnation and blood stasis, it will promote the occurrence of stones. Stones are tangible objects, which in turn block the flow of Qi. If Qi is blocked, it causes pain, so severe pain is common. In addition, stones can easily damage blood vessels and cause hematuria, which can lead to blood stasis over time. Therefore, whether before or after the formation of stones, qi stagnation and blood stasis play an important role in the onset of stones.

(3) Yang deficiency and weak Qi, unable to transport and transform. The kidney controls water. Kidney Yang deficiency has no way to evaporate and transform, and kidney Qi deficiency has no way to push. Stones remain for a long time, water channels are blocked, and kidney Qi is lost day by day, which can eventually lead to kidney decline and separation of Yin and Yang.

(IV) Spleen and Kidney Deficiency The spleen is responsible for transporting and transforming water and dampness, while the kidneys are responsible for the body's water. Stone obstruction and retention of water and dampness can often affect the spleen and kidney functions. After a long illness, the nature of the disease changes from real to virtual, and spleen and kidney deficiency is often prone to occur. If the spleen and kidneys are strong, it will help to drive away evil spirits.

2. Treatment based on syndrome differentiation:

1. Damp-heat accumulation sometimes causes sand and gravel in the urine, difficult urination, or sudden interruption of urination, stabbing pain in the urethra, frequent urination, urgency, abdominal cramps, turbid or yellow-red urine, reddish tongue, thin yellow or greasy tongue, and slippery or fine pulse.

Treatment principle: Clear away heat and dampness

Prescription:

Lygodium japonicum, Lycopodiella ciliata, Pyrrosia ovata

Plantago asiatica

Achyranthes bidentata, white peony root, raw rhubarb, and bitter orange

2. Qi stagnation and blood stasis cause soreness, distension and stabbing pain in the waist, or even unbearable colic, with pain in the flanks and abdomen, and radiating to the lower abdomen or sacrum and coccyx; after the low back pain, hematuria may be seen, which is light red or dark red, with occasional blood or blood clots; the tongue is light red with a thin white or thin yellow coating, and the pulse is deep and stringy or thin and rapid.

Treatment principle: Activate Qi and blood circulation

Prescription:

Winter Melon Seed, Vaccaria Semen, Achyranthes Bidentata, Angelica Sinensis

Red Peony Root Gardenia Citrus Aurantium

Magnolia officinalis, Curcuma zedoaria, Peach kernel

3. Yang deficiency and weak Qi, inability to transport and transform. The waist feels heavy, sore and swollen, cold and painful, dull complexion, cold extremities, fear of cold, no thirst, scanty and white urine, pale and fat tongue with white and moist coating, and slow and deep pulse.

Treatment principle: Warming Yang and promoting diuresis

Black aconite root, cinnamon bark, radix rehmanniae, yam

Poria cocos, Alisma orientalis, Achyranthes bidentata, Plantago
