(I) Primary pruritus Primary pruritus is not accompanied by primary skin damage and itching is the main symptom

(ii) Secondary pruritus

Secondary pruritus is caused by primary diseases and various skin diseases, accompanied by obvious specific skin damage and primary lesions. Pruritus is often a symptom of the primary lesion. Anal pruritus caused by anal fistula, anal eczema, condyloma, neurodermatitis, anorectal tumors, pinworms, etc. belongs to this category.

Anal itching is mild at first, and there is no obvious change in the anal skin. It is mostly paroxysmal and long-term. Patients with itching have severe itching and it lasts for a long time, especially at night. Excessive scratching or mechanical stimulation causes hyperplasia, thickening and roughening of the perianal skin, deepening of the anal folds, local scratches, blood scabs, and exudate. Fecal dirt remains in the folds. In more serious cases, there may be infection, with pus bubbles or purulent secretions, redness and swelling. The lesions can extend to the perineum, scrotum, female vulva and even the skin of both buttocks. Clinical examination can reveal internal hemorrhoids, external hemorrhoids, mixed hemorrhoids, anal fistulas, or laboratory tests can reveal diabetes, pinworm disease, Candida albicans infection, etc.

It is not difficult to diagnose this disease based on the typical history of anal pruritus combined with clinical symptoms and signs, but it is more difficult to determine the cause. Generally, there is a primary disease in the anus that turns into secondary pruritus, otherwise it is primary pruritus. In addition, a full body examination should be performed, and necessary laboratory tests should be performed in a targeted manner, such as routine blood, urine, and stool tests, liver and kidney function tests, urine sugar, blood sugar, glucose tolerance tests, and biopsy and smear tests.
