Buttocks hurt after defecation

The pain in the buttocks after defecation is the pain in the anus. There are many reasons for anal pain. For example, when you have diarrhea, some of the liquid excreted from the stool is very hot and contains bacteria that interfere with the anus and cause a burning sensation. In addition, the patient may have anal fissures, anal inflammation, hemorrhoids or perianal infections. These are all reasons that can cause anal pain after defecation.

What diseases should be considered for pain related to defecation

(1) Anal fissure: The characteristic pain is burning pain in the anus at the beginning of defecation, which is more severe after defecation than before defecation and lasts for several minutes to several hours.

(2) Strangulated internal hemorrhoids: During defecation, a mass protrudes from the anus and cannot retract, causing severe and persistent pain, and there is a history of internal hemorrhoid prolapse.

(3) Anal sinusitis: The pain is mild and usually disappears on its own within a few minutes after defecation.

What diseases should be considered for pain not related to defecation?

(1) Perianal infection: persistent pain and increasing distension in the anus or anal canal, often accompanied by inflammatory manifestations such as fever and increased white blood cell count.

(2) Thrombosed external hemorrhoids: The patient suddenly feels a spherical lump at the edge of the anus, which is purple or black in color. It is caused by rupture of the perianal veins and is accompanied by persistent severe pain, which worsens when walking or standing.

(3) Anal canal cancer: There is often mild persistent stinging in the anus. In the late stage, the general condition is poor and a hard lump can be felt in the anal canal.

(4) Foreign bodies in the anal canal: such as chicken bone fragments, bamboo thorns, fish bone thorns, etc., which enter the anal sinus and cannot be discharged, causing sudden severe pain in the anus. The pain is continuous and worsens during defecation. The patient generally does not have blood in the stool or inflammation. The diagnosis can usually be confirmed by digital rectal examination (sometimes done under local anesthesia).

Spicy food

Excessive intake of chili peppers, garlic, mustard, liquor, etc. will cause irritating ingredients in the stool and may also cause anal pain and discomfort.

For example, cytomegalovirus (CMV) ulcerative proctitis is a common disease among homosexual men. The main symptoms are anorectal pain, burning sensation, anal itching, mucus and blood in the stool, tenesmus, etc. Endoscopic examination can reveal rectal mucosal ulcers, ranging from superficial punctate lesions to deeper confluent ulcers. The entire ulcer is covered with light yellow and white wound membranes. Biopsy and viral culture of tissue from multiple bites can confirm the diagnosis. Surgeons must also be familiar with the manifestations of these diseases in the anus and rectum and early diagnosis techniques.
