Tongue ulcers or tumors: In the early stage, there is only thickening of the mucosal tissue on the lateral edge of the tongue, white spots or small nodules, which gradually form ulcers or tumors and may invade the midline and floor of the mouth.

1. It often occurs in the middle 1/3 of the lateral edge of the tongue, and manifests as ulcers or infiltrates.

2. There are often chronic irritation factors in the corresponding part of the tumor, such as residual roots, residual crowns or poor restorations; there may also be precancerous lesions such as white spots.

3. The pain is obvious and may radiate to the ear, temporal area and half of the head and face.

4. When the tumor infiltrates the lingual nerve and hypoglossal nerve, there may be tongue numbness and tongue movement disorders, and difficulties in speaking, eating and swallowing.

5. Lymph node metastasis is more frequent and occurs earlier, with the deep cervical lymph nodes being the most common. Distant metastasis may also occur, with the lungs being the most common.

6. Generally, tongue cancer invading the floor of the mouth and mouth floor cancer invading the tongue can be distinguished based on medical history.

Tongue cancer often presents with ulcers and infiltration, accompanied by spontaneous pain and varying degrees of tongue movement disorders. In the late stage of tongue cancer, it can directly exceed the midline or invade the floor of the mouth, as well as infiltrate the lingual periosteum, bone plate or bone of the mandible. It can extend backward to the root of the tongue or the anterior pillar of the pharynx and the lateral wall of the pharynx. At this time, the movement of the tongue can be severely restricted and fixed, and the saliva increases and overflows, and it cannot be controlled. Eating, swallowing, and speaking are all difficult. The pain is severe and can be reflected to half of the head. The lymph node metastasis rate of tongue cancer is relatively high, usually around 40%. The most common metastatic site is the deep cervical lymph node group. In the late stage of tongue cancer, lung metastasis or distant metastasis to other parts may occur.
