1. Pedicle torsion About 10% of ovarian tumors are complicated by pedicle torsion, which is a common gynecological acute abdomen. Pedicle torsion is common in teratomas and is prone to occur when the body position changes suddenly. After torsion, bleeding occurs in the tumor, causing the tumor to increase rapidly. Due to obstructed blood flow, the tumor may necrotize and be prone to secondary infection or rupture. The typical symptom of acute torsion is sudden unilateral lower abdominal pain, which will be relieved or aggravated with changes in body position. It may be accompanied by nausea, vomiting and even shock. Gynecological examination can touch a mass with obvious tenderness and high tension, and there is muscle tension. Sometimes the torsion can be reset by itself, and the abdominal pain will be relieved. Once pedicle torsion is diagnosed, surgical treatment is required as soon as possible.
2. The incidence of rupture is about 3%. It can rupture spontaneously or under external pressure. The severity of the symptoms depends on the nature of the cyst and the amount of cystic fluid flowing into the abdominal cavity. It may only cause mild abdominal pain, or it may cause severe abdominal pain, nausea, vomiting, and sometimes lead to internal bleeding and shock. Examination may reveal abdominal tenderness and rebound pain, and the original mass cannot be felt or only a deflated mass can be felt. If there is a suspicion of mass rupture, laparotomy should be performed immediately to remove the tumor and thoroughly clean the abdominal cavity.
3. Infection is rare, mostly caused by adhesion of the tumor to the intestinal tract after twisting or rupture, or spread from the infection focus of adjacent organs. Clinical manifestations include abdominal pain, fever, and a tender mass that can be touched. Treatment is surgical resection of the tumor after appropriate control of infection. It is less common, mostly secondary to twisting or rupture of the tumor pedicle. The main symptoms are fever, abdominal pain, elevated white blood cells and varying degrees of peritonitis. Infection should be actively controlled and surgical exploration should be performed at an appropriate time.
