Add or subtract according to symptoms: For severe heat, add 9g of honeysuckle, 9g of forsythia, and 30g of dandelion to enhance the effect of clearing heat; for lower abdominal distension and pain, add 9g of Chuanlianzi and 9g of Linderae scutellariae to regulate qi; for hematuria, add 12g of Rehmannia glutinosa and 30g of Imperata root to stop hematuria; for oliguria, add 15g of Zexie, 12g each of Poria cocos and Poria cocos to promote diuresis and eliminate dampness.

2. TCM treatment of acute pyelonephritis: Heat stagnation in Shaoyang meridians: clearing the liver, promoting bile secretion and clearing stranguria

Prescription: Xiao Chaihu Decoction combined with Longdan Xiegan Decoction with modifications.

Medication: 9 grams of Bupleurum, 6 grams of Gentiana, 9 grams of Scutellaria, 9 grams of Gardenia, 30 grams of Plantago (wrapped and decocted), 12 grams of Alisma, 3 grams of Akebia, 30 grams of Talcum, 15 grams of Rehmannia, 9 grams of Angelica, and 3 grams of Licorice tip.

Add or subtract according to symptoms: for severe flank pain, add 9g of Corydalis yanhusuo and 9g of Chuanlianzi to soothe the liver and regulate qi; for bitter taste in the mouth and constipation, add 9g of raw rhubarb to clear heat and promote bowel movements; for more severe urinary pain, add 9g of Phellodendron chinense, 15g of Commelina communis, and 30g of Taraxacum mongolicum to clear damp heat in the lower part of the body.

3. TCM treatment of acute pyelonephritis caused by damp-heat blocking the middle part of the body: clearing away heat, removing dampness and relieving stranguria

Prescription: Sanren Decoction combined with Daochi Chengqi Decoction with modifications.

Medication: 9 grams of almond, 6 grams of bamboo leaf, 6 grams of white cardamom, 9 grams of pinellia, 9 grams of magnolia bark, 15 grams of coix seed, 30 grams of talc, 3 grams of akebia, 6 grams of white tongcao, 15 grams of plantain seed (wrapped and decocted), 15 grams of raw rehmannia, 9 grams of scutellaria, 3 grams of coptis, 9 grams of phellodendron, and 3 grams of licorice tip.

Add or subtract according to symptoms: For constipation, add 9 grams of raw rhubarb to clear heat and relieve constipation; for loose stools, add 15 grams of yam and 15 grams of poria to strengthen the spleen and stop diarrhea. For those with abdominal distension and stuffiness, add 12 grams of Zhishi and 6 grams of Chenpi to regulate Qi.

4. Heat accumulation in the lungs and stomach (heat is heavier than dampness):

Symptoms include high fever, sweating, thirst, difficult urination, stabbing pain during urination, a strong or weak pulse, and a dry yellow tongue coating. The treatment should be to clear away heat and detoxify, invigorate Qi and promote the production of body fluid. The prescription can be Wuwei Xiaodu Yin combined with Huanglian Jiedu Tang with modifications. The medicinal ingredients include 10 grams each of Coptis chinensis, Phellodendron amurense, Gardenia jasminoides, 15 grams each of Scutellaria baicalensis, Lonicera japonica, fresh bamboo leaves, 30 grams each of Taraxacum mongolicum, Viola yedoensis, Liuyi powder (wrapped and decocted), Scrophularia ningpoensis, 6 grams each of Akebia trifoliata and Licorice. Decoction in water.

5. Liver and gallbladder heat (evil invades Shaoyang):

Symptoms include alternating chills and fever, restlessness and nausea, bitter and dry mouth, loss of appetite, pain in the waist and lower abdomen, frequent and urgent urination, rapid and stringy pulse, and yellow tongue coating. The treatment should be to harmonize Shaoyang, promote diuresis and relieve stranguria. The prescription can be Xiao Chaihu Decoction combined with Zhuling Decoction with modifications. The medicinal ingredients include 30 grams each of bupleurum, grass, dandelion, plantain, and stone reed, 15 grams each of scutellaria, umbellate, and poria, 10 grams of coptis, and 30 grams of Liuyi powder (wrapped decoction).
