In the early days, people believed that this phenomenon was caused by the local anesthetic flowing to the opposite side through the expanded retroperitoneal space, producing contralateral anesthesia. It is also related to the air injection speed, amount, drug injection speed and length of time lying on the side.

Another group of scholars believe that it is caused by the diffusion of the drug solution from the epidural space of the intervertebral foramen to the opposite side. The ink was injected into the intermuscular space from one side of the corpse. The dissection revealed that the ink was distributed in the intermuscular space of the psoas major muscle on the same side and spread upward and downward along the psoas major muscle. No spread to the contralateral side through the retroperitoneal space was found. After opening the spinal canal, it was found that the blood penetrated into the epidural space through the lumbar intervertebral foramen, and also diffused from the contralateral intervertebral foramen. It was confirmed that spread from the intervertebral foramen was the main cause of bilateral nerve block. It is related to individual anatomical differences.
