Not all lesions with the word "cancer" in their names are malignant tumors. Precancerous lesions are a typical example. In the clinical practice of gastroenterology, many patients, when they learn that they have a certain digestive system disease, especially stomach disease, will ask me in a panic: Doctor, is this a precancerous lesion? If so, am I going to get cancer? From a scientific point of view, the word "pre" in "precancerous lesions" can prove that this type of disease is not cancer, but a disease before cancer or a disease that is prone to cancer. Take gastric cancer as an example. Its occurrence requires an evolutionary process, which is often explained clinically in five development steps: the first step is chronic superficial gastritis; the second step is atrophic gastritis, with glandular atrophy and thinning of the mucosal layer in the stomach; the third step is intestinal metaplasia and atypical hyperplasia; the fourth step is carcinoma in situ, which is basically early gastric cancer; the fifth step is invasive carcinoma, which is in the middle and late stages of gastric cancer and may even spread. It can be seen that gastric cancer is not a one-step process, but a gradual development. Most of the precancerous lesions we are talking about are in the second and third steps. As long as the disease can be controlled, it will basically not develop into gastric cancer. Of course, we must also be vigilant for this type of disease and do a good job in treatment and care. Second, gastric ulcer
The most common group of gastric ulcers is between 40 and 50 years old. The pathogenesis is that the mucosa of the inner wall of the stomach is damaged. The internal part that loses the protection of the mucosa is stimulated by gastric acid, food, etc., and ulcers occur. Ulcers are prone to occur in areas such as the gastric angle, gastric antrum, and cardia. Helicobacter pylori is one of the pathogenic factors of gastric ulcers. It is also a Class I carcinogen. If gastric ulcers are not properly controlled, it is easy to cause intestinal metaplasia and even gastric cancer. In addition, gastric ulcers may also cause gastric bleeding and acute perforation, which are very life-threatening. Third, intestinal metaplasia
Early gastric cancer is closely related to intestinal metaplasia, so in the development of gastric cancer, intestinal metaplasia is the third step, and carcinoma in situ (early gastric cancer) is the fourth step. Simply put, intestinal metaplasia refers to the infiltration of gastric mucosal epithelial tissue by small intestine or large intestine tissue under the action of certain factors, forming small intestine or large intestine type intestinal metaplasia. At present, clinical medicine believes that incomplete and large intestine type intestinal metaplasia have a higher rate of canceration than small intestine type intestinal metaplasia, and are more closely related to gastric cancer. 2. If you suddenly lose your appetite for a long time in a healthy state, often have bloating in the abdomen, or feel full after eating a little, you should immediately undergo a gastroscopy to check whether there are lesions in the esophagus, stomach, or even intestines. For patients who have been diagnosed with precancerous lesions, if they have obvious loss of appetite and weight loss, they should immediately undergo gastric cancer screening, because it is likely that the tumor is causing obstruction, or cancer cells are destroying healthy cells and causing physical abnormalities. |
