"In an emergency, treat the symptoms." When dampness and heat become the main contradiction, the main focus should be on clearing away the dampness and heat, supplemented by strengthening the spleen and promoting digestion.

Wen Dan Tang comes from "San Yin Ji - Bing Zheng Fang Lun" and is composed of tangerine peel, pinellia, poria, immature bitter orange, bamboo shavings, roasted licorice, ginger, and jujube. Originally, "It is used to treat insomnia after a serious illness due to weakness and restlessness, which is caused by coldness in the gallbladder. It can also treat palpitations." The warmth or coolness of its medicinal properties depends entirely on the amount of its ingredients. The prescription is simple in composition and mild in nature. Through flexible addition and subtraction, it can be applied to all syndromes of deficiency or excess, cold or heat, exterior or interior. Its clinical application far exceeds the scope of syndromes and treatments originally recorded. The teacher often uses this prescription with modifications to treat spleen and stomach damp-heat syndrome. The Erchen Decoction in the prescription can dry out dampness, resolve phlegm, regulate Qi and harmonize the middle. Pinellia ternata dries dampness and resolves phlegm, harmonizes the stomach and reduces counterflow, eliminates lumps and resolves nodules; dried tangerine peel regulates Qi and harmonizes the stomach, and the flow of Qi transforms dampness; Poria strengthens the spleen and eliminates dampness; Licorice harmonizes all the medicines, replenishes Qi and harmonizes the middle; ginger and jujube strengthen the spleen and harmonize the stomach, making the earth strong to overcome dampness; Citrus aurantium descends Qi, removes stagnation, eliminates accumulation, eliminates lumps and assists in transforming dampness; Bamboo Shavings are sweet and cold, reduce counterflow, harmonize the stomach, clear away heat and eliminate phlegm. Together they have the effects of eliminating evil, strengthening the body, transforming dampness and clearing heat, regulating Qi and removing stagnation. Although it has the intention of strengthening the spleen, it does not cause stagnation, and it can eliminate dampness without increasing heat. Because damp-heat syndrome is lingering and difficult to cure, clinical practice requires careful examination of the symptoms and the cause, distinguishing the severity of dampness or heat, and flexibly adding or subtracting medicine. The common symptoms of spleen and stomach damp-heat syndrome are abdominal distension, fullness and discomfort, nausea and poor appetite, dry mouth with no desire to drink or drinking little, sticky taste in the mouth, sticky and uncomfortable stool or constipation, pale red or red tongue, yellow and greasy tongue coating, and stringy, rapid or slow pulse. If dampness is more serious than heat, add Atractylodes lancea, Patchouli, Coix seed, Cardamom seed, and Acorus calamus; if dampness and heat are both serious, remove ginger and jujube, and add Scutellaria baicalensis or Coptis chinensis, and Artemisia capillaris; It is rare that heat is more serious than dampness, so do not use too much coldness. In the prescription for both dampness and heat, add Phragmites australis to clear heat, promote fluid production, and facilitate urination, and add radish seed and scorched areca nut to eliminate heat through defecation. In case of indigestion, add Jiao Sanxian and Radish Seed; in case of liver depression and qi stagnation, add Bupleurum, Sophora flavescens and Magnolia officinalis; in case of obvious spleen deficiency, add Atractylodes and White Atractylodes. For damp-heat in the spleen and stomach caused by exogenous infection, warming the gallbladder and intestines is combined in the prescription for syndrome differentiation and treatment, treating both the exterior and the interior. Patients with spleen and stomach damp-heat syndrome often find that there is a lot of retained fluid in the stomach with great viscosity during gastroscopy. At this time, it is necessary to use Atractylodes lancea in large quantities to dissipate dampness with its aromatic properties, invigorate the spleen and aid digestion.

It should be noted that heat belongs to Yang and dampness belongs to Yin. Cool medicines are used to clear heat, as coolness will increase dampness. Warm medicines are used to treat dampness, as warmness will increase heat. In addition, medicines for treating dampness are mostly fragrant and dry, which can easily consume body fluids. Therefore, careful identification is required in clinical practice, and the proper use of medicines must be achieved. Weak spleen and stomach are the source of dampness, and dampness often occurs first and then turns into heat. Clinically, it is rare to see dampness being more serious than heat, that is, dampness and heat are both serious. Treatment is mostly focused on removing dampness and regulating qi, because dampness is most likely to block the qi, and regulating qi helps to remove dampness. The most common herbs for removing dampness are Agastache rugosa, Cymbopogon citratus, Amomum villosum, Acorus calamus, Poria cocos, Coix seed, etc., which are aromatic, mild, penetrating and mild. After the dampness is gone and the heat is isolated, heat-clearing drugs can be added appropriately.

Treatment method: Regulate Qi and strengthen the spleen, eliminate dampness and clear away heat. Wen Dan Tang with modifications. Prescription: 10g dried tangerine peel, 15g processed pinellia, 30g poria, 10g bamboo shavings, 10g patchouli, 15g atractylodes, 12g magnolia bark, 12g calamus, 20g coix seed, 10g scutellaria, and 6g licorice. After taking 4 doses of the medicine, the fullness and stuffiness in the upper abdomen were relieved, and the thick greasy tongue coating became thinner. Patchouli and bamboo shavings were removed from the previous prescription, and 15g each of scorched hawthorn and Shenqu were added. Atractylodes was replaced with Atractylodes 15g. After taking more than 10 doses, all symptoms basically disappeared, food tasted fragrant, but the appetite was still small, the stool was soft and small, and there was still a thin white slightly greasy coating in the center of the tongue. Xiangsha Liujunzi Pills and Baohe Pills were changed to consolidate the therapeutic effect.
